5. Background Check Acknowledgement
To exercise due diligence to ensure that children are protected, we require background checks for adults who meet any of the following situations: volunteer in any capacity, regularly attend events where children will be present, reside in a home that will host events. If a background check has been completed in the last two years, you are current and NOT required to complete one this year.

We are using a Third Party provider for this and will not be storing your sensitive information. If the background check is not done, you will either have to pay the opt-out fee or we will remove your family from the group. If there is an issue with your background check, we will contact you for an interview to discuss.

Will you or another adult be completing a background check this year?

ACTION: Please complete the Background Check Form for EACH ADULT who has not been background checked by LVCHA in the last two years and will be around students and/or LVCHA events in any capacity. Payment Instructions

New Families: Please wait to submit payment until your membership has been approved in the system on July 24. Because membership approval cannot be guaranteed, payments made before approval cannot be refunded through the website. Once you receive an approval email, payment must be submitted within 7 days, no later than July 31.
